- Original high school diploma
- Higher Secondary Certificate
- Original Graduation Diploma or equivalent examination along with mark sheet in original issued by the University
- Scheduled Caste/Scheduled Tribe/Other Backward Classes Certificate
- Physical Disability Certificate (PwBD) in the prescribed format issued by the Competent Authority
- COMMITMENT “B”/SELF-DECLARATION by the applicant for persons with a reference disability (hearing impaired only)
- Certificate of ex-serviceman
- Two passport size photographs (4cm X 5cm approx) along with four additional copies of the same photograph
