The consent form discloses that a behavioral assessment conducted by Concentric will assess applicants’ suitability for employment at Gates Ventures by covering areas such as drug and alcohol use, medical history and psychiatric history related to the job. The form gives permission to disclose the results, including “highly sensitive information,” to Gates Ventures employees.
